Appointment Preparation Checklist
To help make your visit with Dr. Faye as seamless and efficient as possible, please review the following before your appointment:
✅ Bring your insurance card and a valid photo ID.
✅ All of Dr. Faye's patients who are prescribed opioid medications, you are required to bring in your medications to the clinic for every appointment including empty bottles..
✅ Gather any relevant medical history or previous records you’d like to share. If you have had any MRI's or CT's in the past two years, you must bring in a CD with images.
✅ Write down any questions or concerns to discuss with Dr. Faye.
Due to the high demand and personalized individual care plan, there may be a delay in your appointment time. We truly appreciate your patience and understanding.
